DOWNLOAD THE ENROLLMENT FORM HERE. This will start you on your way to using the Affordable Plus Mortgage, First-Time Homebuyer Program, or to obtaining counseling.

Completing the Enrollment Form - It Only Takes About 10 Minutes. Submit the following items to the AHC office. Upon receipt of your information, a Housing Counselor will call you within 2-3 business days to schedule an appointment.

• Completed enrollment form
• Completed and signed credit report authorization form (on reverse of enrollment form)
• $18 credit report fee per individual or married couple, or $36 for an unmarried couple, cash or money order only
• 30 days of pay stubs for all adults age 18+ (excepting students)
