Pre-Qualification & Application Process

Whether you are ready to apply for financing, simply want to see if you qualify, and/or are considering attending counseling, submit the documentation needed to AHC. A loan counselor will call you in about two days to discuss. Feel free to give us a call to discuss your situation first.

  1. A completed & signed AHC's Homebuyer Program's Application Form. Download and print, OR fill it out right online and save on your computer.
  2. 30 days pay stubs for all adults age 18+ (except full-time students).
  3. $18 credit report fee per individual or married couple. Cash, money order or credit card only. Download here for credit card authorization form.
  4. Return all information to AHC.

Although not required, we strongly encourage you to speak with an AHC representative before submitting an application for a home improvement loan. Call 847/263-7478.

  1. A completed & signed AHC's Home Improvement Program's Application Form. Download and print, OR fill it out right online and save on your computer.
  2. Gather and bring copies of the following items to your appointment:
    • Last 2 years tax returns, including W-2s for all borrowers (3 years if you are self-employed).
    • Last 30 days pay stubs for all borrowers.
    • Proof of child support or maintenance.
    • Proof of income if you are receiving income from any of the following sources: pension, retirement, disability, unemployment, workman's compensation or public assistance.
    • Bank statements for the prior 2 months (ALL pages) for all checking, savings and other liquid asset accounts.
    • List all unpaid liabilities and debts, including your account number, and the name and address of the creditor on application (use additional sheet when applicable).
    • Copy of your Owners Title policy, and/or Warranty Deed to the property.
    • Copy of current homeowner's insurance policy.
    • Social security card for all borrowers.
    • Current mortgage statement(s) from ALL mortgage lenders on property (including any home equity loans).
    • Current Lake County tax Bill.
  3. Return all information to AHC.
